Scheduling & Billing
An evaluation may be scheduled by contacting our office directly. If you have been referred to us by your treating physician, we recommend that you contact us directly to schedule an appointment; we will work to obtain the referring records at that time.
We are not contracted with insurance companies, and payment is due in full at the time of your initial appointment and is accepted in the form of cash, personal check, or Zelle. We can provide you with a superbill to submit to your insurance carrier after the completion of the evaluation and upon request. Please note, we do not communicate with insurance companies or complete paperwork on behalf of your insurance company or third-party carriers.
You are welcome to bring a bag lunch if you wish to take a lunch break. There are restaurants nearby should you prefer to leave the building for lunch.
